People who can live off investments or businesses likes mail order or consulting can do quite well in unfashionable old mill towns where housing is cheap and congestion nonexistent.

They need to widen their horizons

Mark Steyn left something important out in his replyy to you Crank. When he mentioned the important shortage of OBs (rather big when you consider dying in childbirth, formerly scarily frequent) is not because of a low birth rate, at least not in the US--maybe in Japan. It's the high lawsuit rate. And remember, OBs are also GYNs, and with more women becoming doctors, there is a gender preference to specialize in it if reasonable. However, the top MDs are becoming opthamalogists, dermatologists, then radiololigists. The hours are good, the interaction with bad news less, and the pay excellent. Who would want to become an OB/GYN in the current climate where you can't bat an eyelash without being sued, your insurance is more than your income, and you owe hald your life to insurance companies (the sports agents of the medical world).
